This is just majorly spitballing, but how them being fraternal twins, so that we don't need to play too much with the who's older and who's younger despite a family ollie out, and while he had trouble, she would have tried to support him, which lead to her avid interest in the 50s version of psychology... which would eventually lead to her being his nurse at the asylum, which she would have thought was for his own good.
That suits the sort of eeriely amoral but genuine love she's meant to have anyway.
